The SSW's Susan Westgate presented at the Health Quality Innovators conference on the subject of addressing the Social Determinants of Health through strategic community-based care coordination efforts.
Westgate emphasized key themes of leveraging community-based insights; population-based vulnerability; and the importance of incorporation of Motivational Interviewing and Trauma-informed Care modalities of care in practice.
