
Susan C. Westgate, Clinical Instructor, participated in a panel discussion at the American Case Management Association’s MD Chapter event on the importance of creation of Post Acute Collaboratives as a way for health care systems to respond to the Triple Aim.
Westgate highlighted the importance of leveraging the right sources of big data while monitoring for population-based concerns. Westgate emphasized that population needs and vulnerabilities stemming from the social determinants of health are often missed in the formation of system-based quality health initiatives.
